Who we are

Support that starts with the person, not the paperwork.

Sylverstone Care Ltd, trading as Prosperity Housing Group West Sussex, is a supported living provider based in Crawley, working with adults across West Sussex who need care and support to live well at home. The people we support are tenants in their own homes; our role is to provide the right level of support, agreed with each person, so they can live safely and as independently as possible.

We are registered with the Care Quality Commission to provide personal care, and we work closely with local authorities, health services, families and advocates so that support is always planned around the individual. Our promise runs through everything we do: empowering independence, honouring dignity, and enriching lives.

How it works

Getting started is simple and unhurried.

Care and support should never feel like a leap in the dark. We keep the first steps human, clear and led by the person.

1

We listen

We take time to understand the person, what matters to them and the life they want to lead.

2

We assess

A co-produced assessment with the person and the people who know and support them.

3

We plan together

A support plan built around the individual and agreed with them — never imposed.

4

We support

The right level of support to live safely, well and as independently as possible at home.

What we do

From a little support to a lot — shaped around you.

We provide care and support in people’s own homes. Our current focus is supported living for adults with mental health needs, and we are registered to grow into wider support as our team develops.

Our current focus

Supported living

We support adults with mental health needs to live in their own tenancies, with support shaped around each person — from daily living, medication and appointments to budgeting, keeping safe and staying connected to the community.

Registered service

Domiciliary care

We are also registered to provide domiciliary care in people’s own private homes. We will offer this as the service grows and our staff are trained and assessed as competent to deliver it safely.

All ages

Adults 18 and over

We support younger adults aged 18 to 65 and older adults aged 65 and over, with support planned around each person’s needs, goals and wishes.

Who we support

Registered to support a range of needs.

We are registered with the Care Quality Commission to support adults across a range of needs. Our current, active focus is supporting adults with mental health needs — and we extend to other groups as our team’s training and experience allow, so we only take on support we can deliver safely and well.

Our promise

Empowering independence. Honouring dignity. Enriching lives.

Empowering independence

We help people build skills, confidence and control over their own lives and their own homes.

Honouring dignity

We treat everyone with kindness and respect, we listen, and we support people to be heard.

Enriching lives

Support is co-produced with each person and built around what matters to them, their strengths and their goals.

Person-centred

Every plan is agreed with the person — support fits the individual, not the other way around.

Safe and accountable

We follow the Mental Capacity Act 2005, safeguarding good practice and CQC standards.

Connected

We work alongside local authorities, health services, families and advocates around each person.
